Preheat the oven to 180°C (350°F). Line a loaf pan with parchment paper.
In a large bowl, combine ground beef, ground pork, breadcrumbs, milk, eggs, finely chopped onion, minced garlic, Worcestershire sauce, ketchup, Dijon mustard, salt, and black pepper. Mix until just combined.
Transfer the mixture to the prepared loaf pan and press it evenly. Bake in the preheated oven for 45 minutes, or until cooked through and set.
While the meatloaf bakes, prepare the caramelized onions. Heat olive oil in a skillet over low heat, add the thinly sliced onions, and cook, stirring occasionally, until golden and soft, about 30 minutes.
In a small saucepan, combine tomato paste, apple cider vinegar, honey, and a pinch of salt. Cook over low heat, stirring frequently, until the tomato glaze thickens, about 5 minutes.
Once the meatloaf is ready, let it rest for 10 minutes, then slice into thick slices.
Slice hoagie rolls in half lengthwise and butter the inside of each roll. Toast the rolls in a skillet or under the broiler until golden.
To assemble, spread the tomato glaze on the bottom half of each toasted hoagie roll. Layer with slices of meatloaf, caramelized onions, and mozzarella cheese.
Place the assembled sandwiches on a baking tray and bake in a 200°C (400°F) oven for 5-7 minutes, or until the cheese is melted.
Serve hot with extra tomato glaze on the side for dipping, if desired.
