In a medium saucepan, combine heavy cream and peppermint extract. Heat over medium heat until the mixture is hot, but not boiling. Remove from heat and stir in vanilla extract.
In a mixing bowl, whisk together the egg yolks and 1/2 cup of granulated sugar until the mixture is light and slightly thickened.
Gradually whisk the hot cream mixture into the egg yolk mixture, a little at a time, to temper the eggs and prevent curdling.
Strain the custard mixture through a fine-mesh sieve into a clean bowl to remove any lumps or cooked egg bits.
Divide the custard mixture evenly into 4 ramekins.
Place the ramekins in a large baking dish and pour hot water into the baking dish until it reaches about halfway up the sides of the ramekins.
Carefully transfer the baking dish to the preheated oven and bake for 35–40 minutes, or until the custards are set but still slightly jiggly in the center.
Remove the ramekins from the water bath and let them cool to room temperature. Then, cover with plastic wrap and refrigerate for at least 4 hours, or overnight.
Before serving, sprinkle about 1 tablespoon of granulated sugar evenly over the surface of each custard.
Using a kitchen torch, caramelize the sugar by melting and browning it until it forms a crisp golden crust. Alternatively, place the ramekins under a broiler for 1–2 minutes, watching carefully to prevent burning.
