Preheat your oven to 375°F (190°C) and line a loaf pan with parchment paper or lightly grease it with olive oil.
In a medium saucepan, combine the lentils and water. Bring to a boil over medium-high heat, then reduce the heat to low and simmer for 20-25 minutes, or until the lentils are soft and the water has been absorbed. Set aside to cool slightly.
While the lentils cook, heat the olive oil in a large skillet over medium heat. Add the onion, carrot, and mushrooms, and sauté for 5-7 minutes, or until the vegetables are softened and the mushrooms release their moisture.
Add the garlic to the skillet and cook for an additional 1-2 minutes, stirring frequently. Remove from the heat and set aside.
In a large mixing bowl, mash the cooked lentils with a fork or potato masher, leaving some texture intact for added bite.

Spread the remaining 3/4 cup of ketchup evenly over the top of the loaf.
