In a large skillet, heat 1 tablespoon of butter and 1 tablespoon of olive oil over medium heat.
Add the thinly sliced onions, 1/2 teaspoon of salt, and 1/2 teaspoon of ground black pepper. Cook, stirring occasionally, for 15-20 minutes or until the onions are soft, deeply caramelized, and golden brown. Remove from the skillet and set aside.
While the onions cook, divide the Italian sausage into 4 equal portions and form them into flat, round patties slightly larger than the bread slices. The patties will shrink slightly as they cook.
