Peel the onions and slice them into thick rings, about 1 cm wide. Keep the rings intact as much as possible.
Peel and finely chop the garlic cloves. Set aside.
Heat a large cast-iron skillet over medium-high heat. Add 2 tablespoons of olive oil.
Place the onion rings in the hot skillet in a single layer. Let them cook undisturbed for 3-4 minutes or until charred and caramelized on one side.
Flip the onion rings and char the other side for an additional 3-4 minutes. Remove the onions from the skillet and repeat with the remaining onion rings as needed.
Bring a large pot of salted water to a boil. Add the spaghetti and cook until al dente according to the package instructions. Reserve 1 cup of pasta water before draining.
Return the skillet to medium heat and add the remaining 2 tablespoons of olive oil and 2 tablespoons of butter. Add the chopped garlic and cook for about 1 minute, stirring frequently, until fragrant.
Add the charred onions back into the skillet. Stir in the crushed red pepper flakes, a pinch of salt, and some freshly ground black pepper. Cook for 2 minutes to combine the flavors.
Zest the lemon and juice half of it, reserving the other half for later. Add the lemon zest and juice to the skillet.
Add the drained spaghetti to the skillet along with ½ cup of the reserved pasta water. Toss everything together to coat the pasta in the sauce. Add more pasta water as needed to achieve the desired consistency.
Remove the skillet from heat and sprinkle in the grated Parmesan cheese. Toss until the cheese is melted and incorporated into the sauce.
Finely chop the parsley and sprinkle it over the pasta. Taste and adjust seasoning with additional salt, pepper, or lemon juice as needed.
Serve immediately, garnished with extra Parmesan cheese and a drizzle of olive oil if desired.
